Fort Worth ISD Student Athletes Can Receive Free Medical Exams 

So, the Fort Worth ISD and John Peter Smith Hospital are teaming up to make the requirement a little easier -- and free. 

Any incoming 7th through 12th grade Fort Worth ISD student interested in playing sports during the 2009-2010 school year is eligible to receive a free comprehensive medical examination on Saturday, August 1, 2009.  These exams will be conducted by John Peter Smith Hospital physicians and specialists at the Wilkerson-Greines Activity Center, starting at 8:00 a.m.   

Dr.  Darren Kuman, cardiologist, and Dr. Ann Ranelle, ophthalmologist, are leading the team of JPS family medicine residents, nurses and other health care professionals who are volunteering their time for the one-day event. 

Transportation will be provided from each Fort Worth ISD high school to Wilkerson-Greines on the day of the exams. Drop-offs and pick-ups at the examination site will not be allowed. All students must ride the bus and bring the physical exam form with them. Because of privacy concerns and space, parents will not be allowed to wait inside the building where the exams are being conducted. 

Any student who wishes to participate should first pick up an official UIL physical form at the FWISD Athletic Department’s Farrington Field Office or at Wilkerson-Greines Activity Center during regular business hours. The UIL pre-participation evaluation/medical history form must be filled out and signed by a parent/guardian.  The physical exam cannot be performed unless the Medical History form is signed by a parent or guardian. (The actual pre-participation evaluation and physical examination form will be filled out by the physician during the exam.  Please do not bend or fold this document.) 

Students should wear athletic gear such as shorts, t-shirts and athletic shoes and, if applicable, glasses or contact lenses.  Female students should wear a sports bra or tank top beneath their t-shirt.   Do not wear caps, jewelry, flip-flops or sandals.  Students: do not bring cell phones.
