FWISD AGAIN OFFERS FLU SHOTS TO EMPLOYEES 

Fort Worth, Texas (January 7, 2013) – The Fort Worth ISD will again offer flu shots to all employees and their family members who were unable to take advantage of last fall’s clinics. If you missed getting a flu shot, it’s still not too late! 

Vaccinations will be offered by Passport Health – North Texas Flu Shots on Wednesday, January 23, 2013 from 3 p.m. to 6 p.m. in the lobby of the Central Administration Building, 100 North University Drive and at Wilkerson-Greines Activity Center, 5201 C.A. Roberson.

We will have all the same vaccinations we had at our flu clinics. If you received your first Hepatitis B vaccination at your campus during the flu clinic, it is time for your second dose. You will need a third and final dose five months or more from now.  You can ask your personal doctor if they can provide this or wait until next fall to have it with your flu shot.

On the date of your vaccinations you must bring a photocopy of your TRS ActiveCare insurance card as well as a copy of your driver’s license to receive your vaccinations at no charge. 

After you register, you will receive a confirmation containing a link to a consent form.  YOU ONLY NEED TO PRINT OUT ONE CONSENT FORM. All vaccines are on the same form this year, except B12 and Zostavax, which require additional forms. Please bring these filled out along with your photocopies.  This will ensure the shortest possible wait. 

If you need a record of the vaccines you received from us in the past, please send an email to info@northtexasflushots.com.  You will need to include your name, TRS ActiveCare Insurance ID number and the email address you would like your records sent to.  Please do this at least five days before you are going to your clinic to ensure we have ample time to provide your records to you.
