Service Records
Service records are official State documents that serve as verification of a full-time employee’s service and State personal leave information.
There are two main reasons why you might need a service record. When requesting a record, please complete the appropriate form below and return to Employee Records.
- To verify experience for graduate work or certification >> Request form
- To verify the number of days/years you have worked for Fort Worth ISD if you are going to work for another school district >> Request form
Things to know about our service record process:

A year of service must be complete before you can receive your record for that year.

HR receives hundreds of requests for service records each year. We complete record requests in the order they are received and as quickly as possible.

If requesting a record at the end of a school year, you should allow a minimum of 60 days from your last day of service for receipt of the record. This is to ensure that all days worked and leave days used are correctly recorded.

If you are going to a work for another District, you may want to inform them of our process so they will not be expecting your record before we can provide it. Most districts understand this – they know what is involved in providing service records.

If you need us to complete a form from another State or private school, etc. verifying your Fort Worth ISD service, please do NOT write in the Employer section of the form that you send us. That is our responsibility. Additional information you want to provide us should be included on a separate page.

Usually records are mailed to the requestor’s home address. On the front of the envelope, it will note: Requested material – open immediately. Please watch for this envelope.
