Affidavit of Residency
An Affidavit of Residency is designed for parents who are enrolling student(s), don’t own or lease property in Fort Worth ISD, but reside with a person(s) that does. Provided the resident agrees to sign an Affidavit of Residency attesting that the parent and student live with them, the student is eligible to attend school in the attendance zone where the resident lives. “Parent” includes a guardian, conservator, or other person having lawful control of a student.
The following steps must be followed to obtain an affidavit of residency:
- Both the parent and the Fort Worth ISD resident must be present to obtain the affidavit of residency form from the attending school’s Principal
- The Fort Worth ISD resident MUST provide residency verification using a current utility bill (not exceeding 60 days from current date), current lease agreement, deed, tax record, or notarized affidavit of residency to the Principal or Principal’s designee
- The Affidavit of Residency MUST be signed by Principal or Principal’s designee before the document is notarized
- Once the Affidavit of Residency has been completed and signed by the Principal or Principal’s designee, the signatures of the parent and Fort Worth ISD resident must be notarized. Notary services are available at the Student Records Office if a notary is not available at the school.
Affidavit of Residency
The parent and Fort Worth ISD resident MUST have VALID STATE ISSUED ID for the affidavit of residency to be notarized.

All Affidavits processed are for the current school year. New school year requests will be processed after August 1st.
The parent or the educational guardian may void an Affidavit at any time. This must be completed at the Student Records Office. A valid state issued ID is required.
