Itemized Statement of Enrollment History
An Itemized Statement of Enrollment History provides demographic information for a student. The Student Records Office will provide the Statement to:
- the parent/guardian, provided the student is under 18 years of age, or is over 18 but is financially supported by his parent/guardian
- the student, provided the student is at least 18 years of age.
These statements include the following information:
- student's name
- date of birth
- student ID
- schools attended
- dates of enrollment
- grade in school
- guardian's name
The earliest enrollment records available in the Student Records Office are for the 1972 - 1973 school year.
An Itemized Statement of Enrollment History that contains information at one school level is $2.00.
An Itemized Statement of Enrollment History that contains information at more than one school level is $5.00.
Note: Records with a privacy code or other restriction in place may not be able to be released. Consult the Public Information section or contact the Student Records Office for further information.
