The Service Record Packet is provided to assist you in documenting previous work experience.  Depending on the system for which you are requesting credit (public or private schools, college or universities, follow one of the criteria below to document your school experience.

It is the responsibly of the newly hired employees to contact former employer to request and submit the following Official Texas Service Records (form FIN 115) for salary increment purpose and unused state days will transfer to the next school district.  Documents should be received in a timely manner no later than the last day of the school year hired (employee’s current work schedule.)   

 

Texas Public Schools and Charter School

 Official Texas Service Record (form FIN 115) 

 

Texas private school, Out-of-State private/ public schools

Submit the following three completed forms:

• Official Texas Service Record (form FIN 115) with official authorized signature, stamped, contact information name, title, physical mailing address, phone number, and personal email address.

• Accreditation statement using form Verification of Accreditation for Salary Increment
• Verification of Expererience for Private  Schools and/or Out-of State Public Schools Verification of Experience

 

College or University 

 Submit the following three completed forms: 

Official Texas Service Record (form FIN 115) with official authorized signature, stamped contact information printed name, title, physical mailing address, phone number, and personal email address.

• Accreditation statement using form Accreditation Statement
• College Verification of Experience (Full-Time) form
• College Verification of Experience (Part-Time) form

Service Records are produced: "within" 30 days from the day of your request, OR "within" 3 weeks after you receive your last paycheck from FWISD, whichever is later. (Payroll must complete processing your last check and State Personal Leave days).
