Immunizations

  • Parents and Guardians,

The Fort Worth ISD is here to help you check school immunizations off your back-to-school list.

FWISD is partnering with Immunization Collaboration of Tarrant County to host Back-To-School shot clinics for uninsured families at Diamond Hill-Jarvis High School July 31- August 12.

The clinic is open from 9 a.m. to 6 p.m. daily and from 9 a.m. to 2 p.m. on Saturdays.

Immunizations are $8 per vaccine for children ages 0-18. Only cash, checks, CHIP (Children’s Health Insurance Program), and Medicaid are accepted. No one will be turned away for an inability to pay for immunizations. Families with private insurance are encouraged to contact their primary physician to receive the required school immunizations.

Diamond Hill-Jarvis High School is located at 1411 Maydell St. in Fort Worth.
