The Rank One Sport Online Forms for 2023-2024 are available. After reading the information below, please click the link above to access the required student-athlete participation forms.  

Please read the instructions on the home page carefully. Download and print the UIL official Medical History/Physical Exam form to take with you to your health care provider. All Fort Worth ISD athletes entering the 7th - 12th grade must have an ANNUAL physical examination. The physical must be dated after May 1, 2023 to be accepted for the 2023-2024 school year. All physicals will expire at the end of each school year, no matter the date of the exam. 

High school student-athletes give your completed physical form to your athletic trainer. Middle school students give your completed physical form to your middle school coach of your first sport. A new physical will be required for the 2023-2024 school year. 

The Electronic Participation Forms to complete for the 2023-2024 school year are the Medical Emergency Card and the Insurance/UIL Signature Page.

After completing these steps, student-athletes are now ready to participate in the Summer Strength/Conditioning Programs & Sport Specific Skill Instruction and in Athletics for the 2023-2024 school year.
