Records
Special Education records are maintained by Fort Worth ISD and provided to school districts upon request when a student enrolls in a Texas school or in a school in another state.
Parents, legal guardians and/or adult students may request special education records through the Fort Worth ISD Special Education Records Office. Copy of photo identification or verification of identity is required along with the Request for Release of Confidential Information form.
For students currently enrolled in Fort Worth ISD and receiving Special Education services, contact the campus the student attends to receive a copy of special education records.
Notice of Records Destruction
Attention Parents/Guardians, Former Students, Eligible (Adult) Students: The legal retention period identified by the Texas State Library & Archives Commission (TSLAC) for Special Education records collected related to the identification, evaluation, educational placement or the provision of Special Education in the district is: cessation of services plus 5 years. Special Education records include referral data, notice/consent documents, assessment reports with supporting data, ARD committee deliberations, and IEP documents.
Special Education services end when the student no longer is eligible for services, graduates, completes his or her educational program, or moves from the district. FWISD has a practice of maintaining Special Education Records from the date of birth of the student plus 26 years (age 21 +5). This notification is to inform parents/guardians and former students of FWISD's intent to destroy the Special Education records of students with a birth year of 2000 or earlier prior to the beginning of the 2024-2025 school year.
Parents/guardians of these students or the adult student (aged 18 or over) may contact the Special Education Department of FWISD at 817-814-2860 before June 30, 2025, should they wish to obtain a record scheduled for destruction.
