2024-2025 Sick Leave Program

Sick Leave Program (SLP) request forms must be received by the 10th of each month, except for short payroll periods (submission dates will vary).  December Submission Deadline: December 2, 2024.

Applicants must use the following request forms to apply:

The FWISD Sick Leave Program cannot be responsible for late, incomplete, inaccurate or undeliverable request forms. Request Forms must be submitted to the Health Services Department.  

Request Forms not completely filled out and/or with inaccurate information by the applicant and/or doctor will not be processed, delaying the process. Applicants are solely responsible for ensuring request forms meet program deadlines.  Request Forms will not be marked received until the request forms are complete, accurate and received by the Health Services Department. 

The Committee cannot be responsible for an employee's lack of knowledge of the program as a reasonable effort is made to notify employees of the benefit.  

Decisions by the Committee to disapprove an application for use of the Sick Leave Program, or an application for extension of such use, are final with no rights of appeal.

Retiring or Resigning?  Please consider donating your unused local sick leave by sending an email to talent.management@fwisd.org with the following information: Subject Line: "SLP Donation".  Please include Name, EID#, Location Name/Number and Resignation/Retirement Date.
