Fort Worth ISD Surveys
Information for Parental Consent
General Districtwide Surveys (3rd through 12th Grades)
The Fort Worth Independent School District (District) periodically administers student surveys to understand and support our students. The surveys provide an opportunity to learn more about student perceptions, opinions, and ideas and to identify specific needs. Some surveys are administered to obtain specialized funding and comply with state and federal law (e.g., the High School Sports Student Interest Survey).
By participating in these surveys, students can have a voice in their education.
- Most of these surveys are anonymous. However, where anonymity is not possible, results are kept confidential and only shared per FERPA and other applicable state and federal laws to protect students' privacy.
- All student surveys are strictly voluntary. Students who do not participate will be given an alternative educational option to complete during the survey. There are no consequences or penalties if a student does not participate.
- Students may skip any questions they do not wish to answer and/or may stop taking a survey at any point.
For additional questions or concerns, please contact askeval@fwisd.org.
The CDC's Youth Risk Behavior Surveys (9th through 12th Grades)
The Youth Risk Behavior Survey (YRBS) is a national survey conducted by U.S. Centers for Disease Control (CDC) that collects data from students in grades 9–12 from both private and public schools within the U.S. The survey captures the health-related behaviors and experiences of youth including: student demographics, health behaviors and conditions, substance use behaviors, and student experiences.
Data from these surveys help monitor adolescent health behavior changes over time, identify emerging issues, and plan and evaluate programs to support youth health.
- These surveys are anonymous and are designed to protect the student’s privacy. No personally identifiable information is ever collected or reported.
- These surveys pose little to no risk, however, some students might find certain questions to be sensitive.
- These surveys are strictly voluntary. Students who do not participate will be given an alternative educational option to complete during the survey. There are no consequences or penalties if a student does not participate.
- Students may skip any questions they do not wish to answer and/or may stop taking the survey at any point.
You can review additional information and copies of these surveys on the CDC's YRBS websiste. For additional questions or concerns, please contact FWISD Health and Physical Education.
