FWISD to Offer Onsite Flu Vaccinations Jan. 17

The Fort Worth ISD will offer onsite flu vaccinations for employees from 3 to 6 p.m. January 17 at the District Administration Building, 100 N. University Drive. 

Passport Health/North Texas Flu Shots will administer the shots.

The following vaccines are available:

Quadrivalent Flu Shot    

With Insurance: No charge   

No insurance: $26


Tetanus Diphtheria with Pertussis    

With Insurance: No charge   

No insurance: $70


Hepatitis B 

With Insurance: No charge   

No insurance: $80                                                 


Pneumonia (Pneumovax23) 

With Insurance: No charge   

No insurance: $100            


Pneumonia (Prevnar 13)

With Insurance: No charge   

No insurance: $200



With Insurance: No charge   

No insurance: $125                                    


B12 Shot               

With Insurance: $18   

No insurance: $18

Most insurance plans cover immunizations 100 percent. However, a small group of plans do not cover immunizations.

After signing up online, employees will see a confirmation page.  There will be a link to print out the consent form for vaccinations.  Employees only need to print out one consent form.  All vaccines are on the same form with the exception of the B12 injection.  B12 will require a separate form.  Employees with questions about any particular vaccination, are encouraged to see your nurse on the day of the clinic. 

The following insurance plans are accepted:

  • TRS-Aetna ActiveCare 1-HD and 2 Plans – covered at 100 percent
  • Blue Cross Blue Shield – with the exceptions of Blue Cross Gold, Silver or Bronze plans
  • Aetna – with the exceptions of Aetna Gold, Silver or Bronze plans
  • United Health Care 
  • Cigna
  • Medicare Part B (Flu shots only)

TRS Aetna ActiveCare Select plans with group numbers that start with 866344 and Scott & White plans cannot be accepted.  We are unable to accept HMO plans.  *TRS SELECT plans with group numbers starting with 866344 must see a Baylor Scott & White physician for all services, including vaccinations for the plan to cover the services.

On the date of the vaccinations, employees must bring a photo copy of their insurance card as well as a photo copy of their driver’s license to receive vaccinations at no charge.

Cash or checks made out to North Texas Flu Shots will be accepted by employees who are paying for vaccinations.

If you received vaccinations from North Texas Flu Shots last year and do not have a copy of your records, please email  In the email, include the email address you want the records sent to, your name, date of birth and your insurance ID number.  Please do this at least a week before your scheduled flu clinic to ensure there’s ample time for sending your records.

Depending on the carrier, insurance claims may be filed at North Texas Flu Shots, Community Health and Immunization Services or Staying Healthy Medical Services.
