Flu Vaccine Clinic For Fort Worth ISD Employees
When: Wednesday, December 5, 2018 from 4 p.m. to 6 p.m.
Where: FWISD Board Room, 2903 Shotts Street, Fort Worth
Who: All Fort Worth ISD employees and their families
Summary: According to the Centers for Disease Control cases of influenza are increasing. And influenza-like illnesses are having an impact on classroom absenteeism. But it is never too late to get a flu vaccine!
On Wednesday, December 5, in the Fort Worth ISD Board Room, you and your family will have another chance to get No Charge flu vaccinations.
Passport Health/Staying Healthy Medical Services will be offering vaccinations to FWISD employees and their families who have the following insurance coverage:
- TRS-Aetna ActiveCare 1-HD and 2 Plans – covered at 100%
- TRS SELECT plans cannot be accepted *
- Scott & White Plans cannot be accepted *
- Aetna*
- Blue Cross Blue Shield*
- Cigna*
- Humana
- United Health Care
- Medicare Part B (Flu shots only-please provide a copy of any supplement plans also)
(*TRS Aetna ActiveCare Select plans with group numbers that start with 866344 and Scott & White plans cannot be accepted. Employees on these plans must see a Baylor Scott & White physician for all services, including vaccinations for the plan to cover the services. We are unable to accept HMO plans, Aetna & BC/BS Bronze, Silver and Gold plans and Cigna Local Plus.)

Cash or checks made out to Staying Healthy Medical Services are both acceptable.
If you have questions about any particular vaccination, please see our nurse on the day of the clinic.
On the date of your vaccinations, you must bring a photo copy of both your insurance card and your driver’s license to receive your vaccinations at no charge.
If you received routine vaccinations from North Texas Flu Shots in previous years, other than flu, and need a copy of your records, please email us at info@stayinghealthymedical.com. In the email please include:
- Your email address
- Your name
- Your date of birth and
- Your insurance ID number.
Depending on the carrier, insurance claims may be filed as Community Health and Immunization Services or Staying Healthy Medical Services.
